If I want to model my first scene, do I do everything in max? Or, do I do it piece by piece and import to an engine of my choice? Do I still bake an Ambient Occlusion if I'm going to import to the engine?
Depends on the engine your using and if your going the modular way or not.
If you using UDK or Cryengine and going modular, then it's better to whitebox the environment first which you can do in either your 3D program or in the game editor.
Generally most environments are built in pieces for culling reasons.

Are you going to build a handful of pieces and then stitch them together to build your level.
Keep in mind that modules can be anything from sets of wall and floor panels to entire buildings or just floors like shown above. It isn't locked down to one specific formula.
I was planning on modeling a ufc/boxing gym. I was wondering if I model the whole gym in Max or for example, model ring and bring it to udk, model the bags and bring to udk, etc....
You can model the whole gym in max first and split it later in individual pieces. But before you go into the whole modeling process make sure your scaling is right. Nothing puts you more off than a f.ed up scaling. Try to work on grid but don't force your models on grid, sometimes it just don't work.
