Need help with Custom brush in Zbrush

Hi all,
I created some custom brushes in Zbrush and the resulting mesh is separated into different polygroups instead of one.
I always end up with this problem but don't know where I went wrong.
Could someone help please? I'm a complete noob

why does it matter?

Just go to Polygroups in the tool tabs and click group visible/masked etc
