Making Portfolio, Suggestions?

So I'm making a new portfolio to dump all my art stuff in, and before I go and start making it, is there something I should shoot for?
What makes a portfolio good?
What elements should I add?
What should I avoid?
And if you have any portfolios that you think are good examples (even your own if you have one), then please link them!

Just search for "portfolio", it brings up tons of threads full of tips/things not to do and so on.
