Rig Issue: Mesh falling off bones?

So this model was working fine when I rigged and skinned it previously with an unfinished model, but when I got the new mesh and reattached it to the same set-up, it's been freaking out. I'm not sure if I did something small that I just can't think of that's making this happen.. or if I messed up big, but this is the problem:

When I move the IK arm by itself, the mesh looks just fine.

But when I move the Root, the mesh jumps right off the forearm of the IK arm.

This doesn't happen in FK mode for the arms, so I thought I just skinned the mesh to the wrong bones. I didn't. I've checked and rechecked the mesh and it's skinned to the correct string of bones. Anyone have any idea what I'm doing wrong? Or what I'm overlooking?

    Just a shot in the dark here - Unless your wrist/hand control objects are constrained to a rooted control object [not necessarily a 'root' control object but just something that is ultimately in the hierarchy and those hand control objects are contained to that], they won't move when you move the root and the geometry can do funky things - especially when IK/FK is involved.
