Yep, when I try extruding the cage it goes towards the middle instead of outside.
I somehow got it working. The normals was facing the right way in maya, but when I imported it to zbrush it was reversed. So I reversed the normals, exported, baked normal map.
Exported to cryengine and the normals was still not correct, had to invert the green channel in PS.
Sometimes backface culling in Maya can be deceptive. Instead of using that in the viewport try using Display -> Polygons -> Face Normals to do your sanity checks, that usually will show you the truth of the matter.

I think I got it now though.
Also you can change how the normal maps are baked in xNormal to be Y + or -.
