FINAL
Hello all,
I've decided to make some items for the Heavy, the theme will be 'Mexican'. "But wait," you say, "the Heavy isn't Mexican, this makes no sense." You're right, but when you think about it maybe it's the only thing that will ever make sense and everything else is just wrong.
Anyhow... The three ideas I have are (I haven't thought of any cool names yet) :
The Sombrero
Straw hat with a little bit of team colours thrown in, similar to this minus the weird looking dude:
Taco
Tasty meat filled replacement for the Sandvich:
Shotgun variant
I haven't decided exactly what I'm doing with it yet, was thinking maybe a decorated shotgun, similar to the ambassador, or an old rusty worn one.
I've already been working on the models for the Sombrero and Taco and will post the progress tomorrow.

Buck, any reason why you think it'd suit the pyro more? I thought the Sombrero would fit nicely with the heavy's ammo belt.
Does anyone know if double sided mesh is ok with the source engine?

Pinata stick is a cool idea except it would need new animations, I guess I need to find an item which can be used with the punch animations he already has.
He could bash them into his enemies with the punch animation. Don't give up on the taco though! That's shaping up to be quite the awesome item.
and while spicy may fit pyro better, the taco is clearly in the heavy's domain, and the sombrero works quite well.
Texture is pretty well done bur will probably be making tweaks once I get it in game.
I'll post the shotgun and maracas tomorrow.

I don't really know how maracas would look without custom animations...since heavy uses fist.

The model for the taco looks great so far! You never see anyone pack a taco that full at restaurant... probably because it would be super messy, but I'm drooling just thinking of a taco with that much filling. :-)
I've heard that for sandvich replacements, that you need to do a modified model with a bite taken out of it... I'm not sure if this is actually true, though. I'd love to hear a big, satisfying CRUNCH when Heavy takes a bite of that.
Yup I've got a model with a bite taken out, only problem is when I try set it up for in game it just loads both of them on the top of each other :S. It's not a massive problem though, I can just test each one separately and let valve sort it out.
Is there a tutorial anywhere for particles? I've never looked at them in the source engine.

I still am in fact working on this even though I haven't updated the thread in a long while.
Psyke: Yeah the KGB makes the hands disappear, I found the solution though. The game_items.txt script file had a line in it saying to hide the hands when the KGB is equipped.
SirCannonFodder: Thanks, I first thought of the Mexican heavy idea as a joke with my friend but I think it works really nice with his bandoleer.

Viceroy Bubbles: I like your minigun mockup . I would have liked to do a minigun but I couldn't think of anything that fit the theme.
I've got the maracas in game with custom viewmodel animations, they don't have any spec on them yet which I'll be doing tomorrow.
I'll try capture some animation tomorrow of the maracas in motion, oh and I'm probably going to do a custom taunt for them .
