Oniram is right on the money. If you let go of shift while smoothing zBrush will use a different smoothing algorithm that takes care of poles and pinches much much better.
the absolutely best thing about scultris was it's viewport navigation, followed by its pinch brush and the adaptive tesselation. however i doubt the viewport navigation will make it into zbrush :P
What's wrong with this? It's probably screwing up because you have useless edge loops there. EDIT: There's a little bit of pinching there, but an extra control loop below it would fix that.
