I'd either create a tri parts curve brush in ZBrush, or model them in 3dsmax and use a path deform modifier.

wow thats a really neat modifier. never used it before but it definitely comes in handy for making 2x1 maps. :D

Your normals are all kinds of weird. Throw an Edit Normals modifier on it and down near the bottom of the menu click Reset.

to export the cage u need to use an editable mesh with an projection modifier in max for export. not an editable poly then this should work.

In max, you need to apply a UVW Xform modifier and set the U Tile to 0.5, then apply an Unwrap UVW at 2048x1024.

Maya 2012 has "Modify > Replace Objects" that can do this. Not sure exactly in which Maya Version it was introduced tho.

you can try using the smooth modifier, takes some tweaking and specific object selecting but it works relatively well.

for max there is this script. http://www.scriptspot.com/3ds-max/scripts/image-tracer-script put an extude modifier on it and your done
