Pretty sure you can keyframe the 'visibility' channel on an object, you could try animating that from 1 to 0 over time.

Send all but one of the faces into 1-2 UV space instead of 0-1, then move them back when you've baked it.

Don't have much knowledge about mental ray, but my guess would be changing the light radius to something higher than 0.

If you're having this problem and gamma correction is turned off, try turning it on and then off again. Sometimes the check box lies to you. :0)

Does that portion of the low-res mesh have UVs, and have you tried automatically computing the distance to surface instead of using 0?
