You probably want to use the UVW map modifier rather than unwrap for that. UVW map is mostly for projection work (box, planar, sphere, cylindrical, etc), which then can be followed up with uvw unwrap if needed. If you want it to be procedural..which is the real power of substance designer is to use FX Maps, if not of course you can sculpt or manipulate images to get the height map.(good start for learning FX Maps https://www.youtube.com/watch?v=61TmAi7hoc0&list=PLB0wXHrWAmCwLRTzdb-RxadGk_xBBQKar&index=9)

The CS4 update didn't work for me. There was a marginal performance boost on some brushes at the default canvas size but it's still too screwed up to be useful. There's a thread about it here http://boards.polycount.net/showthread.php?t=61209 It's worth trying out but don't be surprised if it doesn't work out your kinks.
