i dont flatten layers at all. create new layer, "Image > Apply Image" gives a flat image on that layer of all that is visible. or select all > copy merged > paste on new layer. this way you can have a flatten version to work on and the "source" layers all in one file :) ...just group stuff to make things neat oh and great…
In CS4 you have File > Scripts > Save Layers to Files works just fine. Can't you make an action that would create a new layer beneath each folder, run Apply Image, make all other layers invisible and run the Save Layers to Files script with the "Only visible layers" checked? Do you know what I'm getting at? :)
Solved! I exported successfully with Game Exporter without the model have been locked inside a layer. I simply unlocked the layer. It's weird because Game Exporter asks for what content should it export, but the locked layer causes this rubbish to the model.

You can do that example already using 3dsmax's Modifier Stack. The base object of the stack would be a simple mesh, your "layer 1", then you'd add an Edit Poly modifier as your "layer 2". In the Edit Poly modifier, you connect the edge rings to create the supporting edge loops. Now you can drop back down to the base…
Bump: I think this is because you don't have a locked background layer underneath. You can have sample all layers ticked and it'll work on a layer above the flattened, locked layer but it will not work without one. If all your layers are floating it won't work.
