Does it need to be content already exported (or already setup for export) into Unity? Or can it be the raw files (.max and PS?)

To export a log text file go to help>support>export log>save the text file to drive.

If you're using glTF format, the BabylonJS exporter for Blender should allow you to do this automatically on export.

Yes, you can export easily by doing "right-click > Export with dependencies..." on your substance in the explorer.

Zbrush assigns lots of these weird little groups on export. To avoid it, untick the "grp" button in the export options.

It has to be problem with exporting. The model you uploaded has hard edges. Try looking into export settings.

What format are you exporting in? I'm not to familiar with softimage, but it sounds like your not preserving uvws at export.

Chances are, that your FBX export from your software keeps quads but the substance FBX export triangulates.
