To clarify, I would like the lowpoly mesh elements to be tiling and have more complex geometry than what can be represented in a height map.

With some apparent sub surface scattering it looks Elemental quality. Skin tone is prolly one of the best executions I've seen so far.

Attaching just means combining as topologically non-contiguous elements. To attach them topologically you would have to weld the verts/bridge the edges.

The output .obj is fine, it's only the uv's that are split into faces (elements). I can weld/merge them with a tolerance of 0.01, which works too.
