The amount of pixels inside the faces is not proportional to the size of the faces on the real model, so scaling it fixes it a little but not really

On the low-poly object, go into Face sub-object, select all, Hide Selected Faces. Then try again.

Be sure that one of your faces isn't missing UVs - DDO will mention this if even one face isn't UVed.

It doesn't do any occlusion. It will select all faces that are facing the front view, regardless of whether they are behind others.

Corner of her mouths should be flat with the face. They should divot in. Lips wrap around the curvature of the teeth, not the face.
