On one of my uni projects we have a folder of other peoples concept art, but it's a folder marked 'REFERENCE' and has the artists name as a file name for each piece. I would guess that it is all to do with folders. I tested the site in a folder off my sites root that I called tmh. Therefore in the index,shtml, the inlcude file should lose the tmh, and in the stylesheet you'd need to check all the image urls too.
