You absolutely shouldn't have to do this. The cage should only account for the ray projection and nothing else. Sounds like a bug to me. I will have a word with Dalai.
Remap the 3d cursor from Action Mouse to something else and it should work ok :) Type set 3d cursor in the prefs input search and it will find the correct field.

Yeah, most of the tutorials are written by folks who dont know what they're doing at all. Sometimes it feels like blender looks at what everyone else is doing and deliberately trys to be different.

I've not had that, but have had a similar issue: sometimes 'l' for selecting the entire surface stops working for me. Everything else seems fine, but 'l' just stops until I restart Blender.
