Yep at some point the vert table was reordered. You might of attached the bike then detached it which would jack up the vert table. Morphix has gotten me out of tough spots like that in the past, great little tool. You could also try to "Skin Wrap" the new mesh to the old while its in the T Pose, since they're so close it… -

That cave is actually a bit more simple than the dark souls walls and floors to be honest, the cave is most likely 1 or several cave pieces with geometry that matches the silhouette you see there. The pipes and attachment hardware are separate meshes with their own materials. Probably a vertex blended material on the cave… -

Looks great! I'm getting a GlaDOS vibe :D I think the piping coming down and attaching to the forearms seems very uni directional and a bit boring. I would actually have this loop and weave around certain elements, perhaps a few of the inputs hanging out of the arm plug sockets, just to make it a bit more visually…
