Engines support vertex normals of any kind, it's just data ;) They can also be manipulated in realtime via vertex shaders.

Nooo, only pretending beating him up! :P But on the screen the character gets beat up... in realtime... with blood and gibs. ;)

Is this a render of a game model or whats the purpose of this? Why not go for a realtime render instead to get a more accurate "game feeling" to it?
