Pretty much the same as these guys, except I throw a M_ mor materials. T_ for textures. SM for static meshes. -

Looks good except for the DOF. Is that done in post or UDK? It just looks odd because it starts very suddenly and harshly. -

except that reselling through steam (if managed properly) could still allow both valve and the developer to take a cut of the resale. -

except this CTRL-Z changes your Vert/face indexs, invalidating any topology based edits above it in the stack. -
Re: Max - Save Command Panel rollout positions!
Hotkeys or Quads are the way to "solve" this. If the rollouts are annoying, avoid using the command panel at all, except for managing the modifier stack. -
