  you can fix this problem by holding the Alt key when you hit print screen.

  I didn't mean the screen shot, I meant the tear on the faces of my model in 3dsmax. look at the small button, there is a huge tear running up either side of the face I extruded it from.

  Boolean can sometimes cause similar problems. Try removing turbosmooth then converting your mesh to Editable Mesh and then back to Editable Poly.
  
  Or there's an issue with the modelling, can you post a shot without the turbosmooth? Is the bolt thing actually connected to the main mesh? There's no reason why it can't be a seperate element snapped to the surface. It will keep your geometry simple, helping to avoid problems like this.

  Note to side track, but there's actually a really cool tool included with windows (for a while now), called the 'snipping tool'. It will take a full screenshot, an open window, or a rectangle selection. It also has a 'highlight' feature.

  here are few better images that I hope will show the issue better. I'm still confused about this...
  
  I created the geometry by chamfering 1 of the vertices's with 4 lines connected to it, then dragged the result to the top of the geometry it's fixed to. The same was done with other geometry but without issue.
