I see it now. For shits and giggles, try creating a box and attaching your model to the new box to reset it. See if that helps. Otherwise, the only thing I can think of is that you have hidden edges in there somewhere. You can always try checking with STL Check.
if using normal booleans you are requred to have a waaay denser mesh in the begining and spend ages cleaning it up. i suggest you try with proboolean or just cutting this out by hand. i can show you how when i get home.
yeah i once tried using boleans to save myself some time like this too. ended with 2 hours looking for a problem with the model and then just redoing it in 10 minutes oh and for a simple piece like this one you should really keep it in quads
