Workflow for Fully Customizable Characters in Games Production?

Hi everyone!

I'm looking for a workflow which allows players to customize their characters from different skins. Normally I would delete all the faces that we don't see in-game to save polycount, but how about games that have a customizable character system like Dota 2, WOW, Diablo 3, GW2, and Black Desert,..? It seems like they have a base naked body with costumes as separate meshes. I find this workflow is fine with low poly game characters but how about next-gen models? That would be very expensive.
