Why does my manipulator change in Maya??

This is a problem I've been having for awhile and today it finally made me ask WHY THE FUCK!!??

Normally I can translate a vertex in the viewport freehand, when this option is available the yellow box remains a square pointed at the camera when you rotate around the object.

BUT SOMETIMES the yellow box doesnt stay square, it acts as a plane and when I translate a vertex in the viewport it seems to lock to a certain axis making it a bitch to move around.

as I rotate around, the yellow box doesnt turn with the camera:

Hopefully someone can help me out...
