The concelt is clear and easy to follow, considering it does not need proper animatable topology, no realtime model, no bakes and no proper UVs i would say this model would take about a week to be modelled. So it comes down to the daily/hourly rate of the artist you pick. On top you need to add the fees for 3d printing which can totally vary dependent on the size, quality and vendor location. it is hard to say "this will cost you XXX USD" as it depends on a lot of things.
